This page uses the Contact Page template (set it under Page Attributes → Template). The map and contact details come from Appearance → Customize → Business Info.
To add a working contact form, install Contact Form 7 or WPForms, create your form, and paste its shortcode below as its own paragraph — the template will detect and display it automatically in place of the fallback form.
